Operational OR Maintenance Error occurred during an Environmental Upgrade of a 24'' Flare Head Line linking the Peremabiri Flow Station to the Flare Head - about 50M from the Flare Pit. The process was not followed through properly, which lead to discharge of Crude Oil into the Environment.
