The clamp, previously installed to arrest a leak, failed under operational pressure, resulting in a significant release of crude oil. Field observations indicated that the clamp was unable to withstand the line pressure, likely due to material fatigue, seal degradation, or a gradual loss of structural integrity over time. No signs of tampering were observed.
