There was a tear of about 9cm long on the 8"Agbada 2- Nkpoku pipeline. During the initial site investigation, the JIT noticed that there was no recent excavation within the spill point. The horizontal tear was was at 9 o'clock position. The pipeline looked rusty hence the cause of spill is equipment failure.
