unknown persons(s) installed 3'' valves on the facilty on two(2) different points@ a 12 o clock positions and the distance between each points was approximately 4.5cm. two hoses were seen connected to the valves and the spill was within and outside the company's ROW and there was no containment measure put in place as of the time of JIV
