THE CAUSE OF SPILL WAS AS A RESULT OF EQUIPMENT FAILURE SINCE A PIN HOLE WAS SEEN ON THE WELDED SECTION ON THE WELLHEAD. THE AREA OF IMPACT WAS WITHIN THE COMPANY RIGHT OF WAY. VEGETATION WAS IMPACTED
