Oil Spill at 4’’ Azuzuama 5 Flow line at Azuzuama

i. There was a pool of water at the incident point during initial assessment. ii. After excavation the JIT saw a cut of about 130mm long at 1 to 6 o’clock position on the flow line. iii. The point was not leaking during JIV and repairs.
