• The JIT team on arrival observed visible impact of Crude oil on the surface water. • Divers confirmed that the initial spill was due to Equipment failure as a result of leakage from a submerged pipeline crossing the river which was not in use since the introduction of by-pass line. • Separating activities was also carried out at the riser causing the release of crude oil as a result of back flow from the well-head. • Control measure was introduced including river booms, skimmer and containment tank which was put in place to recover the released crude oil and prevent further spread
