1,Divers confirmed that the initial spill was due to Equipment failure as a result of leakage from a submerged pipeline crossing the river which was not in use since the introduction of by-pass line. 2, Separating activities was also carried out at the riser causing the release of crude oil as a result of back flow from the well-head.
