i. During initial assessment the JIV team observed recent activity within the incident point. ii. Upon exposure of the flow line by excavation, the JIT saw a 2’’ ball valve illegally installed at 12 o’clock position on the Flow line by unknown persons. iii. The incident point was not leaking as of the time of JIV.
